Leeds and Somerset share their learning on implementing personalised care and support planning

The Health Foundation has launched Building the House of Care setting out case studies from Leeds and Somerset of their experience implementing personalised care and support planning using the House of Care model.

The case studies are based on interviews with key stakeholders from both health economies including the CCGs, Public Health Team, lay members and Year of Care Trainers.  The publication explores the experience of both areas in detail, and includes a discussion section looking at how their learning can be used to support other health economies.  The discussion explores three questions relating to the House of Care:

  1. How do you embed a new model into a pre-existing system?
  2. To what extent can and should the model be adapted to the local context?
  3. What is the best approach to achieving spread?
