Call for partner sites to strengthen the evidence on person-centred and community-based health and care

The Realising the Value programme, funded by NHS England and led by Nesta and the Health Foundation, is calling for partner sites from the voluntary community and social enterprise sector, or that work closely with this sector, to learn about the barriers and what works in implementing person-centred and community-based health an care.

The programme is seeking five local partner sites to each lead on one of the give areas of practice identified by the consortium

  • Self-management education
  • Peer support
  • Health coaching
  • Group activities that promote health and well-being
  • Asset-based approaches in a health and well-being context.

Local partner sites will receive high quality support to enhance their work and have the opportunity to learn from other experienced sites across the country as well as showcasing their own work.  Sites will need to commit to working with the consortium from November 2015 to June 2016, and the consortium is offering a grant to cover the direct costs of involvement.
