About the person-centred care resource centre
We want a more person-centred healthcare service. One where health and social care professionals work collaboratively with people. We want healthcare services to support people to be better at making informed decisions and managing their own health and care. This requires a change in behaviour and mindset from healthcare professionals and patients - supported by a system that puts patients at its heart. We are working with healthcare services, teams and professionals to help them advance their skills, knowledge and leadership so that people have a better healthcare experience and better quality of life. Over the coming year we will be building recognition and understanding of what a person-centred NHS looks like and sharing evidence and practical tools to enable us to support wider change.
The new resource centre is designed to help healthcare professionals implement a more person-centred healthcare service, where people are supported to more effectively manage and make informed decisions about their own health and care.
About the Health Foundation
