As with many aspects of care, shared decision making and self-management support apply across a range of health conditions and are relevant all along the care pathway. Yet most commissioning - and the way in which services are structured and provided - is still ‘condition specific’.
This gives two possible approaches to commissioning:
- The ‘umbrella approach’ - an overarching strategy that seeks to embed shared decision making and self-management support across services.
- The ‘integrated approach’ - a pathway based approach that seeks to embed shared decision making and self-management support as part of the work in every commissioning programme.
