Agenda and goal setting form (diabetes)
This sheet is designed for someone with diabetes to use with their health professional to help that person think about what they would like to talk about, which goals they want to set and what actions they will take to achieve them.
This sheet was developed by Whittington Health as part of their work on the Health Foundation's Co-creating Health programme.
It sets out a series of health-related behaviours, such as exercising and eating well, and encourages the person to think about how they feel they are doing and areas they would like to improve on. It provides a series of prompts to help the person plan how they can achieve their health-related goals.
