This guide is designed for anyone who has health and care needs over time, or cares for someone who does. It is intended to help people to understand what care and support planning is and how it might help, see how care and support planning works; find out about other people's experience of it; and get started with care and support planning. 

The guide sets out the purpose of care and support planning, who is involved and the four stages of good care and support planning and how people can and should be involved.  The four stages it describes are:

Prepare – getting ready for the care and support planning discussion.  

Discuss – talking with the care and support partner.

Document – writing down the main points from the discussion.

Review – checking on how things are going.
