The COPD patient pathway is an on-line interactive resource designed to help people living with Chronic obstructive pulmonary disease (COPD), their carers and healthcare professionals identify the type of care COPD patients should be receiving, from diagnosis onwards.  It shows how services that support self-management should be integrated into the pathway.

Developed in collaboration with Almirall and based on the very latest guidance on best-practice COPD care, the pathway also includes NICE Quality Standards and real-life best practice case studies from around the UK.

It highlights key elements that need to be in place to support self-management, from a comprehenasive self-management plan, to peer-led self-management training and peer support.


Newly diagnosed with COPD. Health care worker in the past. Limited knowledge, other than supporting patients in care environment. What should I expect of Gp, care team, etc. What support is out there. How do I stand on money issues. Whats the difference in inhalers. Plenty of questions, only don't know where to find answers that I can trust.

After having a persistent cough for over a year, I was diagnosed with COPD in March 2015. In 2016 my COPD got worse to the point where not only do I have trouble sleeping at night, I also struggle to get air into my lungs. I am 59 now. I was diagnosed with COPD in March of 2013. I had been a heavy smoker, got a cold I couldn't get rid of, ended up in the hospital, on a ventilator. I should have known it was coming, but like most smokers, thought it would never happen to me. I don't smoke now and never will again, but, I still want to. My life has changed drastically.
