Developing information booklets for patients
This case study briefly describes how a Cambridge GP practice working with the Health Foundation's Co-creating Health programme has produced a ‘family’ of stapled A4 booklets which contain, as appropriate, agenda setting prompts, results and condition-specific information. Patients can also incorporate goals and action plans agreed with their clinician.
The booklets build onagenda-setting and information sheets which the practice had created for patients with COPD, diabetes and heart problems who were due to attend a review appointment. The intention is that patients can keep the booklets for future reference.
In addition, practice staff have found the development process motivating and have a model which they can extend to other areas.
