This case study describes how the team in Cambridge working on the Health Foundation's Co-creating Health programme ran multi-disciplinary training for a stroke ward, including designing new forms to help them use self-management support approaches.

It describes how the training has led to changes in some of the systems and practices on the ward and the positive impact that has been achieved.
