Islington's approach to integrated care

This paper describes the approach Islington has taken locally to implementing the House of Care and integrated care. 

This includes a commissioning approach focusing on both intensive users of health and social care, and health and wellbeing of the broader population; developing new pathways of care for people with long-term conditions; focusing on self-care, personalisation, patient activation and mobilisation of community assets; aligning urgent care services across centres co-located with A&E, NHS 111 number; working on GP out-of-hours and general practice; and better aligning physical and mental health services.  It goes on to outline some the outcomes that Islington has achieved in terms of outcomes for patients, Health and Care Professionals, systems and organisational processes and commissioning.
