This one page sheet is designed to help people with diabetes set self-management goals and plan how they will achieve them.

With a lively and colourful design, the sheet sets out some suggested health goals that someone with diabetes might set themselves and takes the person through key points in planning how to achieve them, such as when they will do it, and what might potentially stop them, finally asking the person to assess how confident they feel that they can achieve their goal.
