In this article Helen Sanderson explains that a one-page profile answers three questions: what do others like and admire about you, what is important to you, and what is good support for you.  

She describes how developing these simple profiles for each patient provide an entry point into personalised care for each person.  They can help staff get to know patients better, and when similar profiles are created by staff, can also help patients get to know the people who are caring for them.  The article includes some case studies of where this approach has been used.
