The Health Foundation’s Practitioner Development Programme distils the evidence about what works into a clear set of practical skills and tools that clinicians, health coaches and others can learn and use to support people to self-manage.  This series of resources is designed to enable people to set up and run their own practitioner development programme.

Practical guide

This guide provides information to enable the running of practice development workshops, which are the core component of the Practitioner Development Programme, which also includes action learning sets and other support.  The Guide sets how the workshops are structured and run and includes practical information on how to set them up.

Pre-Training Questionnaire

This questionnaire is designed to be completed by participants at the start of the preparatory workshop for the Practitioner Development Programme, to help individuals assess their own starting point and to enable them to track the impact of the programme.

Pre-training delegate pack

This suggests a reading list of articles and information  that delegates may find it helpful to read before starting the Practitioner Development Programme.

Preparatory workshop slide presentation

This presentation is designed to be used in the introductory workshop of the Practitioner Development Programme.  It outlines the challenges for today’s healthcare economy regarding Long Term Conditions (LTCs), supports practitioners to explore their knowledge, beliefs and understanding of Self Management (SM) and Self Management Support (SMS), sets out the evidence base to support this approach and initiates individual and team planning.

Workshop 1 slide presentation

These are the power-point slides for use in the first of a series of workshops that form the programme.  During the workshop participants explore the skills  which contribute to successful collaborative agenda setting and consider how their team and services work and how these processes impact their ability to successfully support patients to self manage. 

Workshop 2 slide presentation

These slides are designed to support the second workshop in the programme. During this workshop participants explore the skills  which support them to understand the patient’s perspective (health beliefs and patient activation, explore patient importance, explore patient confidence, help patients at the beginning of the journey to move forward; exploring ambivalence, support the patient to move forward with goal setting & action planning.

Workshop 3 slide presentation

These slides are designed to support the third workshop in the programme. During this workshop participants explore the skills  which support them to support collaborative goal setting & action planning, support the patient to review how things went, share success, consider what got in the way – barriers, consider how to move forward – problem solve, share information & expertise, explore how they will proactively follow up, identify the support they need to progress.

Workshop evaluation form

This evaluation form has been designed to be used at the end of each workshop to enable the workshops to be 'fine-tuned' and to track their impact.

Handout 1 Self management support and the enablers

This is the first in a series of hand-outs designed to support the programme.  The handout outlines the definition of self management support, its implications for health services and how behaviour change can be supported by using the three enablers of  agenda setting, goal setting and  goal follow up. 

Handout 2 - Goal setting and action planning

This is the second in a series of hand-outs designed to support the programme.  The handout explains what is meant by goal setting and action planning and explores how practitioners can use these approaches in their practice.

Handout 3 - Follow up

This is the third in a series of hand-outs designed to support the programme.  The hand out explores what is meant by ‘goal follow up’ and suggests some questions practitioners can use to support goal follow up.

Handout 4 - Skills descriptors

This is the fourth in a series of hand-outs designed to support the programme.  The hand out identifies and explains the key skills practitioners need to use in their practice to support self management.

Post training questionnaire

This questionnaire is designed to be completed by participants at the end of the final workshop of the Practitioner Development Programme, to help assess the impact of the programme.

How engagement in the programme can support CPD, revalidation and KSF

This document describes how participating in the Practitioner Development Programme can be used towards appraisal and revalidation and how it links to the Knowledge and skills framework for nurses and allied health professionals.

I really enjoyed your resource centre- I am an educator in a LTC/hospice facility in Calgary Canada but I trained as an RN in the UK over 30 yrs ago. I know that the CEO of this company is very interested in this topic so I will send him the info

The recommended site www.health.org.uk/sms for downloading materials is not to be found. I have found the resource centre helpful and I will be taking it to our SMS team.
