Telephone follow-up for COPD patients completing the rehab programme

This case study briefly describes how the team in Ayrshire and Arran working on the Health Foundation's Co-creating Health programme changed services so that people who complete the Pulmonary Rehabilitation Programme are no longer invited to a face-to-face appointment but receive telephone follow-up.

The idea was introduced as around 40% of patients did not attend their follow-up appointment.  The team found that most patients actually preferred telephone follow-up and that the approach also saves clinician time.
