This Delivery Plan for NHS Wales sets out new Welsh Government commitments to the public for diabetes care in Wales up to 2016.  It focuses on supporting children, making education the priority as a means of supporting self-management, and establishing an information system to underpin services.

Supporting people living with diabetes is one of 5 key objectives in the Plan.  The main approach to this is through structured education programmes, although the plan also encourages innovative ways of providing patients with information to support their condition that will include making use of local community based services and voluntary groups, such as Communities First activities. 

The Plan also sets out that Diabetes Specialist Nurses have a crucial role to play in delivering improved care to people with diabetes, both in the community and hospital, and an important educational role and that professionals supporting individuals with diabetes should be trained in psychological techniques which build self efficacy into their clients. Family based interventions around lifestyle change should be supported and encouraged as this can provide the vital social support for sustained change. Also, each patient with diabetes will have an individualised care plan designed around their specific needs.
