The COPD patient pathway is an on-line interactive resource designed to help people living with Chronic obstructive pulmonary disease (COPD), their carers and healthcare professionals identify the type of care COPD patients should be receiving, from diagnosis onwards. It shows how services that support self-management should be integrated into the pathway.
This is part of a series of Smart Guides to Engagement. It helps clinical commissioning groups (CCGs) adopt an approach to developing pathways reflecting what matters to patients, carers and family members. It offers examples that work and points to sources of help to achieve success.
