Academic / research papers
Published in the NIHR Journals Library and featuring research commissioned by HS&DR, this study involved a rapid, systematic overview of the evidence on self-management support in people with 14 diverse, exemplar long term conditions. It is the most comprehensive, systematic study of the area to date.

The teams in Cardiff and Newcastle working on the Health Foundation’s MAGIC programme developed a series of tools to help them understand the progress they had made in implementing shared decision making, the challenges and some of the approaches to overcoming these.

The Talking Points Person Outcomes Framework is an evidence based framework comprising the views of people living in the community who use care services about what is important to them in life and the impact of health and social care services on outcomes. This report considers the reporting and use of personal outcomes information in the context of self-management support and the applicability of the framework within different care contexts and at different levels of decision making.
Tools for managers
The Ask 3 Questions campaign is designed to encourage patients to ask questions and play a more active role in decisions about their treatment and care. There are posters and leaflets and other materials which can be displayed to encourage and support people.
This audit tool provides a simple checklist to help services assess how well they are promoting materials that encourage people to Ask 3 Questions.

The LTC6 Questionnaire asks patients with a long term condition about their healthcare over the last 12 months. It includes questions about involvement in decision- making, although the focus is on self-management. The measure can be used to drive improvements at both the population and provider level.
Tools for managers
This interview template was developed by Cambridge University Hospitals NHS Foundation Trust as part of its work on the Health Foundations Co-creating Health Programme. It sets out guidance and questions for semi-structured interviews with patients to understand how well supported they have felt in setting their own health goals and trying to achieve them.
