It features a series of patients explaining why it is important to be involved and setting out three questions that can help people to understand their choices. It can be used alongside posters and other materials to support an 'Ask 3 questions' campaign.
These materials were created by the Health Foundation’s MAGIC programme and can be adapated for local use. They include a poster, flyer and business card designed to encourage people and increase their confidence in playing an active part in decisions about their own care.
